The Highly Integrated Dual Eligible Special Needs Plan (HIDE SNP) is a Medicare Advantage program designed for individuals who qualify for both Medicare and full Michigan Medicaid. The goal of HIDE SNP is to combine Medicare and Medicaid benefits under one coordinated health plan, making it easier for members to receive care and services without managing multiple insurance programs.

Under a HIDE SNP plan, medical services, prescription drugs, and Medicaid-covered long-term services and supports are coordinated through a single insurance provider. This allows for better communication between providers and improved continuity of care.

Eligibility

An individual may qualify for a HIDE SNP plan if they meet all of the following requirements:
• Age 21 or older
• Enrolled in Medicare Part A and Part B
• Enrolled in full Michigan Medicaid (not partial or spend-down only)
• Reside in a county where HIDE SNP plans are offered
• Not enrolled in hospice services
• Not residing in a state-operated veterans home

Eligibility is determined by the State of Michigan and the participating health plans.

Service Areas – Region 10 and Region 12

HIDE SNP plans are available in multiple Michigan regions. Angels Care of Michigan serves clients in the following regions and counties:

Region 10
• Macomb County

Region 12
• Wayne County

Residents of these counties who meet Medicare and Medicaid eligibility requirements may be able to enroll in a HIDE SNP plan.

Several managed care organizations participate in the HIDE SNP program in Regions 10 and 12. Availability may vary by zip code and enrollment period. Common participating plans in Wayne and Macomb Counties include:

• Priority Health
• UnitedHealthcare Community Plan
