PACE Program

Information about the PACE Program.

What Is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is an integrated Medicare and Medicaid long-term-care program for qualifying older adults. PACE is different from hiring an independent home-care agency — the PACE organization coordinates the participant's overall care through an interdisciplinary care team.

General PACE Requirements

General requirements for PACE participation include:

  • Age 55 or older
  • Meeting applicable Medicaid long-term-care eligibility criteria
  • Living in the service area of a PACE organization
  • Being able to live safely in the community at enrollment

What PACE May Coordinate

Through its interdisciplinary care team, PACE may coordinate:

  • Medical care
  • Personal care
  • Adult day health
  • Therapies
  • Medications
  • In-home care
  • Transportation
  • Hospital care
  • Nursing-facility care when needed
  • Other medically necessary services
Program information can change. Eligibility, services, and authorization are determined by the applicable government agency, health plan, or administering organization.
